Position Summary
Our Clinical Care Coordinator is responsible for assessing comprehensive needs of each of our residents to develop an appropriate plan of care. This involves comprehensive assessments, care conferences, Doctor’s rounds, coordinating resident’s appointments, weekly wound assessments and collaborating with the MDS coordinator with the admission process and the written care plans.

Salary
$72,800-$114,400
Duties / Responsibilities
- Participate in quality improvement
- Audits
- Implement policies, procedures, standards of care, and care plan contents
- Assist with resident admissions and discharges
- Participate in MDH survey process
- Attend/coordinate resident appointments
- Develop care plans based on resident/family goals of care and assessment findings
- Coordinate and participate in provider rounds
- Communicate with staff, residents, families, and/or providers
- Perform resident assessments
- Attend care conferences
- Assist staff on the floor, may include nights, weekends, and/or holidays
- Other duties as assigned
Education / Experience
- Education: Graduate of an accredited school of nursing with current Registered Nurse Licensure by the Minnesota Board of Nursing or have State Border recognition through the Minnesota Board of nursing.
- Holds a current and unrestricted Minnesota Nursing License
- BLS certification required.
Knowledge / Skills / Abilities
- Ability to organize, plan, implement, supervise, direct, and evaluate nursing activities independently.
- Demonstrates excellence in clinical skills and understanding of the Nursing process.
- Must be available to work weekends, holidays, and rotating shifts as needed to ensure 24-hour nursing needs are met.
- Must have computer skills, including, but not limited to Word, Excel, and PowerPoint, experience with electronic health records a plus.
