Available Blocks: • Days 68 hrs/pp (Combo of 5:30a-6p and 6a-2:30p, Every other weekend and holiday) • Eve 56 hrs/pp (2p-10:30p every other weekend and holiday) • Nights 72 hrs/pp (5:30p-6a, every third weekend and holiday) • Nights 68 hrs/pp (Combo 5:30p-6a and 2p-10:30p, Every other weekend and holiday) • Nights 48 hrs/pp (5:30p-6a every third weekend and holiday) Shift Differentials: Evening: $1.50 Night $2.00
Position Summary
The CNA provides direct care to residents while under the supervision of a licensed nurse, RN or LPN. The Care Center CNA provides care for adult and geriatric patients in the Madison Healthcare Services’ skilled nursing facility.

Duties/Responsibilities- Put your nursing skills to work!
- Provides direct care services for residents at the Care Center, including meeting residents’ needs in the following areas:
- Dressing, bathing and grooming
- Food and fluids
- Exercise and walking
- Elimination
- Mental stimulation
- Vital signs and weight
- Filling supplies in residents’ personal care basins in their rooms.
- Assists residents to attain highest degree of functional independence.
- Communicates with residents in a kind and caring manner; listens to their concerns and gathers necessary information.
- Takes immediate action to meet residents’ requests or needs.
- Provides residents with tub bath and nail care.
- Assists Therapy Department as time permits.
- Assists with helping residents eat at mealtimes.
- Listens to reports at the beginning of each shift, clarifies assignment, reviews care plan/Kardex for assigned residents. Suggests change in plan as needed.
- Is aware of appropriate outlined care plan and incorporate them into routine for the shift.
- Observes and reports resident changes to the charge nurse.
- Completes residents ADLs in Point Click Care including appetite, target behaviors, bowel movements, etc.
- Participates in performance improvement teams as requested; suggests recommendation to improve quality of care.
- Attends and actively participates in department meetings. If unable to attend, obtains information from review of meeting minutes.
- Alters practice when need to do so indicated by results of PI/QA monitoring.
- Meets continuing education requirement for registry.
- Attends mandatory in-services.
- Recognizes safety hazards and takes corrective actions; seeks assistance when needed.
- Performs work safely, without causing harm or risk to self, others or property.
Education / Experience
- Current registered or able to register as a Nursing Assistant in the State of MN

Knowledge / Skills / Abilities
- Must be able to effectively communicate in English, especially with the geriatric population.
- Must be able to follow verbal and written instructions.
- Possess good judgment and the ability to make appropriate decisions.
- Must be able to accomplish a large quantity of work with a high degree of efficiency.
- Must be able to work weekends and holidays.
